FYI- your captcha doesn’t work in Firefox on Mac right now-
can’t see the digits- or enter a solution.
Had to use Chrome.

Does it work if you empty your browser’s cache before on firefox?

We tried every form in the latest firefox on Mac. They work as expected. There was a gap of about 30 minutes yesterday where a broken form was online – looking like the screenshot you sent over. It seems like you visited the site in that period.
Then your browser cached the damaged captcha and loads it all time you visit the site again.
This is why you need to clean the cache. Here’s a how to: https://kb.iu.edu/d/ahic#firefoxSecond question: Your purchased the subscription model. If you cancel the subscription (on your account page) it will stop working after a year.
